Welcome to the Community Care of North Carolina Provider Portal!

The Provider Portal application is provided to improve patient care and care coordination for NC Medicaid recipients. Providers in primary care practices, hospitals, and other settings may use this secure portal to access care team contact information, visit history, and pharmacy claims history for their Medicaid-enrolled patients. Population management and quality reporting is also available for CCNC practices.

************************** Transition to NCTracks – NC DHHS new MMIS system **************************

On July 1, 2013, the N.C. Department of Health and Human Services (N.C. DHHS) began the transition to NCTracks, a new Medicaid Management Information System (MMIS). This had the effect of suspending the flow of Medicaid claims data to Community Care of North Carolina (CCNC).

CCNC would like to remind users that many other sources continue to be updated and supply data to CCNC’s Provider Portal, Pharmacy Home, and Informatics Center Reports applications.

When using these applications, be sure to note the "Claims Paid Through" date and "Claims Fill Date Through" in patient records.
  • Example: Patient data displays the message “Claims Paid Through: 6/27/2013” and “Claims Fill Date Through: 1/30/2014”.
  • This indicates that the patient data displayed is based on insurance claims information received through June 27, 2013 – which may not be current – and pharmacy claims filled through January 30, 2014.
Much of the information that practices, and providers, CCNC network users, rely on continues to be available as of February, 2014:
  • Medicaid and Health Choice pharmacy claims data from data consolidator ESI – updated weekly in Pharmacy Home and Provider Portal.
  • Near-real-time inpatient and emergency room Admission, Discharge and Transfer (ADT) data from over 55 North Carolina hospitals, including most major healthcare systems in the state -- this clinical data is updated twice each day in all CCNC applications
  • University of North Carolina hospitals and clinics ADT data, discharge summaries and medications – updated daily, available in all CCNC applications
  • UNC Hospitals, Solstas and Lab Corp lab values –updated daily, weekly, or monthly depending on source in Care Alerts, CMIS, Provider Portal and Pharmacy Home
  • Immunization data from North Carolina Immunization Registry (NCIR) – updated weekly in Provider Portal
  • Medicare Part D pharmacy claims from CMS for Medicaid dual-eligible patients – updated monthly in Provider Portal and Pharmacy Home
  • CCNC continues to add new Medicaid eligibles to CMIS and Provider Portal
  • Medicare, BCBS, Aetna, United Health Care and Scotland Health continue to supply administrative claims at the same frequency for patients enrolled in the Multipayer, First in Health and Scotland Health projects -- available in IC Reports, Provider Portal, Pharmacy Home and CMIS
